Swasthya Sankalp is committed to improving public health outcomes, with a particular focus on Non-Communicable Diseases (NCDs) such as hypertension and diabetes. As part of our efforts to enhance comprehensive primary care services, we are seeking a qualified and experienced individual to serve as the Program Manager for the state. The primary objective of this position is to provide technical and programmatic support in the design, implementation, and monitoring of cardiovascular health interventions, focused on improving control of hypertension and diabetes at the primary health care level in selected geographies in the state. The PM will work closely with State Program Manager for the project (SPM), NCD Nodal Officers and Comprehensive Primary Health Care (CPHC) Nodal Officers of the selected geography to implement strategies for improving screening, diagnosis, treatment, and follow-up care for individuals with hypertension and diabetes at the primary health care level.
